Zoning Application Checklist (PDF) - Filing instructions and list of forms/materials needed to make application to the Board.
Application for Hearing - Form 2 (PDF) - Form used to detail petitioner's name, subject property, title information and brief description of work.
Appeal - Form 2A (PDF) - Form used to appeal decisions made by zoning officials.
Variance - Form 2B (PDF) - Form used to request authorization to use land or locate structure(s) in a manner that is otherwise prohibited.
SPA1 Findings - Form 2C (PDF) - Form used to request authorization to use land or structure(s) for a specific use which is expressly permitted by granting of a special permit.
SPA2 Findings - Form 2D (PDF) - Form used to request authorization to use land or structure(s) for a specific use which is expressly permitted by granting of a special permit - requires site plan review by the Planning Board.

Application	Fee
Residential Variance	$300
Commercial Variance	$300 for the first 2,000 square feet $100 for each additional 1,000 square feet or portion thereof
Special Permits	$300 for the first 2,000 square feet $100 for each additional 1,000 square feet or portion thereof
Application Requiring Multiple Permits	100% of highest fee required by Board of Appeals, in addition each additional permit 50% of required fee
Special Permit(s) and/or Variance(s) - Signs	$300
Comprehensive Permit	$250 per unit, in addition the cost of postage associated with abutter notification and the required newspaper legal notice(s).
